How about if the leash attaches under the neck?

Works great for lots of pet! It can be use a front lead on a harness for your pet. The neck piece on the coat is low enough so the leash is attached above the coat.

The medium fits under chest but it doesn’t cover his back, I’m just wondering if I get a large it will cover but will underneath be too big ?

Underneath is adjustable. But be careful, I got an extra large for my dog and the extra large runs small. It may not be long enough to cover his/her whole back.
